In consideration for being allowed to participate in any way in the US Amateur Soccer Association athletic/sports program, and related events and activities, the undersigned:
1. Agrees that prior to participating, they will inspect the facilities and equipment to be used, and if they believe anything to be unsafe, they will immediately advise their coach, supervisor of such condition(s) and refuse to participate.
2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions, or negligence of others, the rule of play, or the conditions of the premises or of any equipment used. Further that there may be other risks not known to us or not reasonably foreseeable at this time.
3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
4. Release, waive, discharge and covenant not to sue the US Amateur Soccer Association, its affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and leasers of premises used to conduct the event, all of which are herein after to as 'releases', from demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole ore in part by the negligence of the release or otherwise.
CERTIFICATION:
By Checking YES and submitting this online registration application, the player agrees to abide by the laws and rules of the game of soccer as promulgated by the International Football Federation (FIFA), the United States Soccer Federation (USSF), the Idaho State Soccer Association (ISSA), and any properly affiliated league or team which may sanction a competition in which the player chooses to participate. The player further agrees that failure to abide by these laws and rules may result in the revocation of the right to play granted by the acceptance of this registration. The player acknowledges and will assume the risk for any personal injury in which they would sustain before, during or after a game and/or practice, and will not hold liable any team, club, state association, the US Amateur Soccer Association, or the US Soccer Federation.
This online form is used to obtain a permanent registration card for adult soccer in Idaho. Your registration will be permanent as long as you play and maintain current registration through annual renewal. Applications are automatically accepted unless your previous record of play suggests repeated failure to abide by the rules of the game, in which case additional information may be needed, IF THE TEAM FOR WHICH YOU PLAY BELONGS TO A LEAGUE AFFILIATED WITH THE IDAHO STATE SOCCER ASSOCIATION, THEN YOU MUST BE REGISTERED WITH THIS FORM OR YOUR TEAM MAY FORFEIT ANY SANCTIONED GAME WHICH IT MAY PLAY.
